Here is an example app with (hopefully) helpful notes on what to put where. For additional questions, feel free to contact the staff!
General Guidelines: Don't copy and paste any text parts, ever. Not in the personality, not in the powers, not anywhere else. We may run plagiarism checks if we suspect this is the case. We don't like people stealing from us and we won't abide you stealing from others, either. If you are bringing over a character from another site, let us know.
Full name: The character's full name.
Aliases: Any names they are otherwise known by.
Age: How old is the character? "(Unknown" is acceptable in the case of some canon characters.)
Gender: Male, female, or otherwise.
Race: Human, mutant, mutate, or otherwise. There is a helpful guide regarding this here.
Alignment: Hero, Neutral, Villain, or a faction.
Birthplace: Where was the character born?
Current Location: Where is the character now? ("Transient" is a perfectly acceptable answer.)

Closing Thoughts: The main things we watch for are inaccuracies in canon characters and possible Mary/Gary Sues. The latter is easy to avoid; if you're unsure how much Sue is in your character, try taking this Mary Sue Litmus Test. As stated above, we also watch for plagiarists. Possible godmodders are also on the wary list. We try to be drama-free here at the 513, and godmodding and metagaming just doesn't fit well with that.
So, what happens after you post your app? It is reviewed by two staff members, each of whom will give general comments on the app. Once any concerns have been addressed, a final decision will be made, and if the character is accepted, you will be able to play them here. Yay!

Appearance:
Write about what your character's appearance here. You could include such things as build, height, weight, eye color, hair color, skin color, face shape and features, body shape, and so on. Also note possible hairstyles and personal fashion - how the character dresses from day to day, or on certain occasions.
Personality:
Tell us what the character is like on a normal day, or even on a good and/or bad day. Include qualities the character might possess. A great way to do this is by placing them in possible scenarios. You don't have to write out the scenarios here, just note how the character reacts and use that here. That could net a couple of paragraphs right there.
Likes:
This is just a list of random things your character likes. There must be at least five, and the number of likes should be equal to the number of dislikes, for balance.
Dislikes:
Opposite from the above - this is a list of things your character hates. They should be equal to the likes again.
Strengths:
This is a list of character qualities - things you might say about yourself in a job interview. They might mirror the Personality section up above. There must be at least five, and the number of strengths should equal the number of weaknesses.
Weaknesses:
Opposite of strengths - things you would never admit in a job interview unless they forced you to! Again, you might find this similar to the Personality blurb, and the number should be equal to Strengths. These are character weaknesses, so a weakness for, say, chocolate would not count (that would be a Like).
Skills:
These are things your character is good at, and, unlike the next section, can be anything from fencing to basket weaving. There must be at least three, and you should balance the number with that of the flaws.
Flaws:
These are physical or psychological flaws. They can include fears, mental illness, physical disability, scars, or simply things the character should be able to do but can't. There should be at least three, but equal to the number of skills.
Skills and Abilities:
Please write in detail the sorts of superpowers your character has, if any. Describe each in terms of what it can do. For example, if the character can manipulate energy, you should include something about concussive blasts and force fields. Please also include limits, such as how far a concussive blast can go or how long the force field can exist. If you are playing a mutant, consider the level they are (omega, alpha, beta, etc). This will help in identifying some limits too.
You can also include skills here. Unlike the Skills list, these skills should be related to offensive or defensive tactics, such as observation skills, fencing, kickboxing, defensive driving, and such. Something like being a good cook will generally belong on the lists but not here. Include some limits for these as well.
In total, you should have about seven sentences for the abilities and skills, and seven sentences for their limits. We're more concerned about substance, though, so if you come up a little short but have written informatively, it'll be fine. Also, if your character has no useful skills or abilities, just note that briefly.
History:
This should start from birth (or before, it it's relevant) and continue to the present day. Be sure to touch on your character's childhood, schooling, and especially any major life events. This part should be able to serve as a reference for the future, and it should help to explain why your character is the way s/he is.
Sample RP:
This just gives us an idea of your writing style and how you plan to write with this particular character. It does not need to be perfect, especially since you may not have written this character before. But it should give us an idea of what the character is in summation, and it should give you a springboard from which to jump into the game when you're accepted!
